Bone Grafting

Veneers vs. Crown

Veneers and crowns are both reliable restorative solutions that protect the structure of a tooth while refinishing its surface to create a strong, reliable and beautiful smile esthetic. However, veneers and crowns have many differences with regard to their purpose, placement and function.
 
Veneers: Veneers are most frequently used as a cosmetic smile solution. They can be used use refinish the surface of a tooth and correct teeth that are too narrow or short, and cover-up stains, small chips and cracks. While a single veneer can be used to refinish an individual tooth, multiple veneers are most often used to refinish the most visible teeth and provide a smile makeover that’s fully customized and idealized. Like crowns, veneers are comprised of a thin porcelain shells that are fabricated from an impression of the existing tooth site. Veneers are usually much thinner than crowns, but the existing tooth structure will need to be slightly modified for permanent bonding and placement.
 
Crowns: Crowns may be used for cosmetic purposes, but they’re most frequently used to restore the surface of a tooth due to a fracture, severe chip, deep cavity, or following a root canal or other procedure that calls for the modification of the tooth.  Like veneers, crowns are custom fabricated to perfectly restore the appearance of a tooth, in addition to protecting its structure.  Crowns are frequently created from porcelain or ceramic, but they may also be formed from gold, porcelain fused to metal or other material.

Digital X-Rays

Digital radiography is one of the most technologically advanced products available to the medical field today. While traditional systems are effective, there are many features of digital X-ray machines that help create a better overall image of your mouth. Dentures

Root Canal

While the thought of having a root canal procedure usually fills patients with dread, the procedure is a necessary one for those whose teeth become infected in the nerves. Besides causing pain, infections, if left untreated, can become abscesses, which are much more serious and result in a loss of bone mass in the jawbone. Intraoral Camera

Crown Lengthening

A dentist’s first goal is always to save your teeth, which means that they have developed many dental procedures that help prevent tooth extraction. In cases where cavities have decimated a tooth to a point where there is not enough tooth structure left upon which to fix a crown, our office will perform a crown lengthening.

If a tooth has broken below the gum line or if decay has developed underneath an existing crown or filling, crown lengthening allows a new crown to be placed rather than extracting the tooth.
While called crown lengthening, this common surgical procedure does not actually lengthen the crown. It actually lowers the gum line, and it does this by removing gum tissue, bone or both to expose more of the tooth and provide a foundation for the crown.

The procedure is performed with a local anesthesia to numb the area. When the area is fully numb, Dr. Nina Kiani will make cuts to pull the gums away from the teeth and expose the roots and surrounding bone. In some cases removing a little of the gum tissue will expose enough of the tooth for placing a crown. However, it is more likely that some bone will also have to be removed from around the roots of the teeth. After removing the necessary amount of tissue and bone, the area will be washed and the gums stitched.

Bonding

Of all cosmetic and restorative procedures in dentistry, bonding is one of the easiest and most affordable ways to enhance your smile. Called bonding because tooth-colored resin is bonded to your teeth, the procedure is used to repair teeth that are discolored, unevenly shaped or chipped. At our office, we can even use bonding as an alternative to amalgam fillings if only a small filling is required.

Bonding is achieved by applying incredibly strong resin to the tooth in need of restoration. The procedure can usually be accomplished in one visit and allows you to keep most of your tooth’s original enamel, which makes bonding preferable to veneers in many cases. Once the right color resin has been chosen for your smile, Dr. Nina Kiani will slightly abrade your teeth to roughen the surface. Then a conditioning liquid is applied to help the resin adhere. The putty-like resin is then applied to your tooth and molded and smoothed until it is in the proper shape. It is hardened with an LED light. After the bonding material has hardened, Dr. Kiani will trim and shape it even more and then polish it until shines like a tooth. Altogether, the procedure usually takes between 30 minutes to an hour for one tooth.

Home Fluoride Treatment

Fluoride is a mineral that occurs naturally in many foods and water and it helps prevent tooth decay by making the tooth more resistant to acid attacks from plaque bacteria and sugars in the mouth. Every day, minerals are added to and lost from your teeth’s enamel layer through demineralization and remineralization. Minerals like fluoride, calcium and phosphate are redeposited to the enamel layer from the foods and waters consumed. Fluoride is important for infants and children between the ages of 6 months and 16 years. However, adults benefit from fluoride, too.  
While there is fluoride in our foods and water, it can also be directly applied through fluorinated toothpastes and mouth rinses. In most cases, fluoride is administered by the dentist during your appointment. During your appointment, the doctor will apply the fluoride to the teeth in gel, foam or varnish form that contain much higher levels of fluoride than the amount in mouth washes or toothpaste. Because it’s been difficult to find effective, affordable fluoride treatment at a drug store, Dr. Nina Kiani has decided to offer take-home fluoride treatments.
 
This take-home fluoride treatment provides complete coverage and helps protect your teeth from harmful plaque bacteria. When you get your home fluoride treatment, it is quite simple to use:
  • Brush your teeth thoroughly.
  • Place the fluoride gel in the tray. 
  • Place the tray completely over your teeth and gently press the tray to move into place.
  • Leave trays in your mouth for 5 minutes.
  • Use the tray with the fluoride gel in your mouth at least twice a day.
  • Once after breakfast and once before bedtime.
  • Do not rinse your mouth after each use. Expectorate the gel residue completely. Do not eat or drink for 30 minutes.
  • Do not eat or drink with your tray in place.
  • Do not use any chemicals or hot water to clean your tray. Use antibacterial soap on a toothbrush to clean your tray and rinse it out.

Dental Implants

When it comes to replacing missing teeth, patients have a lot of options. None of those options, however, are permanent like dental implants. Implants are widely considered the modern-day solution to missing teeth because they provide an aesthetically pleasing, permanent answer to a serious problem. When a patient loses a tooth, it is important to act quickly to fill the space. Apart from the cosmetic issues, patients who have missing teeth will often suffer from compromised chewing efficiency and tooth drifting. The space left behind from a missing tooth causes the other teeth to drift into that space which can lead to misaligned teeth. Receiving a dental implant is a surgical procedure and patients are put under a general anesthetic. The implant will be secured into the jawbone where it will need to sit for three to six months so the bone can grow around it, this process is called osseointegration. After the surgery, patients will visit us to receive a crown over the implant. This is done so the patient’s teeth do not drift and they can eat normally. Once the implant is secured, your dentist will place an abutment on top of the exposed implant that will support a permanent crown.  Dental implants are the only completely permanent solution to missing teeth.

Scaling and Root Planing

If your teeth are sensitive or your gums bleed when you floss or brush, you might be suffering from gingivitis or gum disease. Patients who come in for regular cleanings are much less likely to suffer from periodontal disease.  If you’re not taking care of your oral hygiene, the bacteria and plaque can advance to gum disease and it becomes more difficult to treat. While gum disease sounds like a scary thing, the good news is that there are treatment options available.
 
Gum disease is the leading cause of tooth loss in the United States. There are many different forms of periodontal disease but all of them can cause serious damage to the mouth. The mildest form of periodontal disease is gingivitis. Gingivitis causes the gums to become red, swollen and bleed easily. The biggest cause of gingivitis is lack of a regular oral hygiene regimen. Digital X-Rays

We believe that early detection is the best way to treat our patients. With that being said, we have adopted digital X-ray technology into our practice. Digital radiography will better allow us to diagnose and treat patients the right way, the first time around.
 
Digital X-ray technology offers many benefits over traditional X-rays. The biggest difference is the amount of radiation it emits, about 80 percent less than traditional machines. While the radiation in traditional X-rays is not necessarily harmful, it’s never a good idea to expose someone to any type of radiation.
 
Another great benefit of digital radiography is the immediacy of it. You don’t have to wait for the image to develop anymore. Right after the image is taken it is immediately displayed on a bedside monitor. This greatly cuts down on patient chair-time. The instant imaging software is a great way for us to “Go Green” because there’s no harmful chemicals used to develop the images and the image itself is not printed so there is no waster there. The image is displayed on an environmentally friendly monitor that files it away with your personal records for future use without printing.
 
In many cases, digital X-rays are able to detect early stages of tooth decay where traditional X-rays would not. The image we get is far more detailed than other imaging software. Catching decay early can prevent cavities, gum disease and other periodontal issues. By getting to these symptoms earlier, our staff is better equipped to stop it now and even prevent it in the future.

Veneers

Dental veneers are perfect for patients hoping to redesign and reshape their smile with a simple, non-invasive procedure. Those with misaligned, chipped or broken teeth are great candidates for this procedure. We provide our patients with the opportunity to repair their smile with dental veneers.
 
There are many types of veneers available. At our practice, we utilize ceramic veneers. Ceramic veneers cover the existing tooth structure. The process by which veneers are installed takes two to three visits. In the first visit, Dr. Kiani will consult with you to determine if you are a candidate for veneers. Once approved, Dr. Kiani will give you a set of options such as what size, shape and color the veneers will be. The molding process will then begin. This process consists of removing half a millimeter of the enamel from your tooth surface so the veneers are flush with the gum line. We will then take a mold of the teeth and have veneers structured at a dental lab. Creation of the veneers takes approximately 10-14 days.
 
Once the veneers are ready, they will be fitted into the patient’s mouth to see if the shade is correct and to gauge the size. The veneers will then be trimmed to assure a permanent fit. We will then thoroughly clean the area and etch the teeth before applying the bonding agent. Once the bonding agent is applied, the veneer will be placed in the mouth and the bonding agent will be activated by a UV light source. The final step in the process involves a thorough inspection of the area Dr. Kiani will examine your bite, remove any excess cement and make any final adjustments.

Teeth Whitening Options

Sinus Augmentation

If you are thinking about a dental implant but the surgeon says that you have low quality bone or not enough bone in the area, you might be a candidate for a sinus augmentation procedure. Implants have to be inserted into quality bone in order to be permanent and for those who have lost teeth or have had periodontal disease, the bone is often damaged. The upper back jaw is usually the most difficult area to successfully place a dental implant because there is often a lack of bone quantity and quality. With a sinus augmentation procedure, Dr. Nina Kiani is able to help you gain the bone structure needed to place a dental implant.
 
In some cases, you will be able to undergo a procedure that will reposition the sinus floor that will create enough space for an implant. Grafting material will be utilized to encourage your bone to grow around the area quickly. This newly grafted material will help stabilize the implant.
 
In other instances, patients might have to undergo sinus “lift” procedure. This procedure uses the remaining bone under the sinus floor to and it is gently “pushed up” to lift the floor of the “dropped” sinus. Periodontal Disease Treatments

The mildest form of periodontal disease is gingivitis. Gingivitis causes the gums to become red, swollen and bleed easily. The biggest cause of gingivitis is lack of a regular oral hygiene regimen. Preventative measures, such as regular brushing and flossing, go a long way when it comes to preventing periodontal disease. Gingivitis is reversible with professional treatment and good oral health practices at home.
Untreated gingivitis can lead to periodontitis. At this stage, plaque will spread and grow below the gum line. Toxins produced by the bacteria will irritate the gums, causing an inflammatory response. This inflammation leads to the breakdown of tissues and bone that support the teeth. Periodontitis also causes the gums to separate from the teeth, forming pockets that become infected. As the disease progresses, the pockets deepen and more gum tissue and bone are destroyed. Eventually the teeth become loose and have to be removed.

Crowns

Endodontic Retreatment

Endodontics is a dentistry specialty that involves issues of the nerves and roots of the teeth. Most commonly, it consists of a root canal treatment. During a root canal, the nerves, roots, and other infected tissue of a tooth is removed, which helps rid it of an infection and saves the tooth.
 
After a root canal, a long-lasting crown is usually placed to save the tooth. Unfortunately, at times some teeth need an additional treatment called endodontic retreatment, which is offered by our New York, NY dentist Dr. Nina Kiani.
 
Endodontic retreatment is necessary following a root canal if the tooth does not heal or the infection is not completely eliminated. If the patient keeps having sensitivity to hot and cold temperatures after a root canal, then it was not successful and needs an additional treatment. Common reasons our patients need the treatment to be performed a second time, include:
 
1. Continuing to have a tooth infection after treatment.
2. Narrow tooth canals that could not be treated properly during the first root canal.

Extractions

There are two main categories of extractions performed in our office: simple extractions and those performed as oral surgery procedures. The simple extractions are done on teeth that are not impacted or deeply hidden beneath the gums. The gums do not need to be cut open to reveal the tooth, and a simple loosening of the tooth from its roots pulls it free.
 
Other teeth, including wisdom teeth, need to be surgically removed which requires anesthesia and a longer recovery time. In both cases, it is a routine procedure done for a variety of reason: the tooth is infected or impacted, a wisdom tooth is pushing against other teeth or causing jaw pain, the tooth needs removed prior to an orthodontic treatment.
 
We provide our patients with the following recommendations after having extractions performed:
  • Have liquids or soft foods for the first few days after the tooth has been extracted.
  • Do not smoke cigarettes or drink alcohol as it affects the healing time.
  • Avoid using a sucking motion, such as through a straw, because it can dislodge the blood clot and cause a dry socket.
  • Take all prescribed medications as directed, including antibiotics and painkillers.

Patients that wake up frequently with ear pain, headaches, or jaw pain are often struggling with grinding or clenching their teeth at night. It is a habit that is automatic and almost impossible to stop.
 
A great way to prevent damage and pain caused by teeth grinding, also called bruxism, is with night guards. These are custom guards made to create a barrier between the upper and lower teeth while sleeping, that last about ten years on average.
 
Patients who grind or clench their teeth at night not only experience pain, but it can be noisy and disrupt their spouse’s sleep, wear down the teeth, and damage their tooth enamel.

Bone Grafting

Bone grafting is a necessary procedure provided right here in our New York, NY dental office when a patient needs a restoration and does not have a proper amount of jawbone. Jawbone needs to be thick, dense and deep enough in order to support implanted devices, particularly a dental implant. If a patient has had a missing tooth for a while, chances are he or she don’t have a proper amount of jawbone. With the bone grafting procedure, we can take bone from another area of the mouth or use cadaver bone fragments.
 
Prior to completing a dental implant procedure, we take a series of digital x-rays to determine how much jawbone is available. If it is enough, we proceed with implanting the titanium metal post where the missing tooth is located.
 
If there isn’t sufficient jawbone, we need to perform a bone grafting procedure first to allow new bone to grow before implanting the post. This helps the new bone support the weight and density of the titanium post so it doesn’t reject it. Inlays/Onlays

Inlays/Onlays
Inlays and onlays are a type of restorative dental procedure that helps to protect the tooth from further decay or damage. It is also known as a partial crown, because the process is very similar to a crown. However, it is more conservative than a crown and involves less removal of healthy tooth structure.
Here at our West 34th Street New York dental practice, we perform inlays and onlays to help strengthen a tooth susceptible to breaking, prevent permanent damage that would need an implant or bridge, and restore its shape as well. The inlay is placed inside the tooth, while the onlay is more extensive and goes on the outside of the tooth.
 
Like with dental crowns, inlays and onlays performed in our office take about 2-3 weeks in total. Here is a summary of the procedure:
 
1. The patient visits our office and has a local anesthetic to numb the area of the tooth. This makes it so the patient is comfortable and does not experience any pain during the procedure.
2. A drill is used to remove the damaged portion of the tooth; this is where the inlay or onlay will go.
3. With the tooth prepared, impressions are made for the inlays or onlays and sent to the lab.
4. The patient returns in about two weeks to have the inlay or onlay bonded to their tooth. This is similar to a crown, though it is not covering the entire tooth.

Wisdom Teeth

Wisdom Teeth
Not everyone gets their wisdom teeth, but many of our patients do see them emerge. Wisdom teeth come in after the last set of molars, near the back of the mouth. They typically appear when patients are in their late teens or early to mid-twenties.
 
In some cases, wisdom teeth don’t cause a problem, but others need to have them removed. Because wisdom teeth are so far in the back, they are harder to clean and prone to more severe tooth decay. This can lead to infection and other problems. They might also grow in crooked, pushing against the other teeth.
 
If the wisdom teeth are pushing against the other teeth causing shifting or pain, or they become impacted, we recommend getting them extracted. This is done under anesthesia so the patient does not feel any pain. It is a routine procedure, but one that needs an experienced dentist and oral surgeon, such as our Dr. Nina Kiani.

Dental bridges are one of many dental procedures performed here in our Nina Kiani, DDS, PC dental office. Bridges help to replace missing teeth by using an abutment where the missing tooth is, as well as crowns over the adjacent natural teeth to create a tether for the bridge and provide more stability.

Apicoectomy

Apicoectomy


Root canals are commonly performed in our dental office, but occasionally, the treatment does not completely get rid of an infection. If this happens, an apicoectomy is performed by Dr. Nina Kiani in our West 34th NY dental office in order to complete the dental treatment.


Since there are so many nerves underneath a tooth, it is possible that not all of the infected tissue is removed during a root canal. If a patient still experiences pain or sensitivity to hot and cold after a root canal, there are still nerves present that need to be removed during the apicoectomy.


An apicoectomy is performed by numbing the area, then making an incision in the gum to reveal the root of the tooth. Infected tissue is removed to clean the area of the gum and the gums are then sutured back up. It is more extensive than a root canal, but only a very small area is treated, so it has a relatively short healing time. Like with root canals, we urge our patients to be careful and not chew on that side of the mouth for a few days until it heals.

Dental Bridges

Dental bridges are an excellent way to replace missing teeth, and offered here in our Penn Plaza New York city practice. A dental bridge is made up on abutments where the missing teeth are and crowns on the adjacent teeth to provide the tether to the bridge. They are completed in about 2-3 visits and give our patients the opportunity to chew and talk normally, regardless of where their missing teeth are located.


The process of getting dental bridges here by Dr. Nina Kiani, DMD is as follows:


1. The patient visits our office for a check-up and exam, and to get impressions made of the adjacent teeth.


2. These teeth may need to be prepped prior to taking impressions, such as filing them down to make room for the crowns.


3. After two weeks when the bridge is fabricated, patients return to our office to have it fitted in their mouth with strong cement.


As long as our patients have adjacent teeth on either side of the missing teeth that can withstand crowns, we are happy to provide them with bridges for their teeth that have been pulled.
